The Mu Sigma Phi Fraternity and Mu Sigma Phi Sorority warmly welcome the newest members of the UPCM community. We congratulate the members of Class 2014 for it is indeed a milestone to be accepted in the premiere medical school in the Philippines, and for confirming your decision to bravely serve the country as part of the Return of Service Agreement (RSA) recently implemented by the College. As part of our yearly activities, we will again hold our Freshman Assistance Program (FAP) to make your transition to the UP College of Medicine easy, simple, and enjoyable as well. Starting off, here is a reminder of the important dates and deadlines for the freshmen:

Baccalaureate Transcripts·of Records, two original copies
- Certificate of Good Moral Character from two different persons, one must be from the Dean of school where the applicant graduated
- Photocopy of NSO Birth Certificate (please present original for authentication)
- Photocopy of Income Tax Return (ITR) of parents (please present original for authentication)
- Photocopy of NMAT result .(please present original for authentication)
- Clearances, if applicable from appropriate Agencies/Offices (e.g. DOST etc.)
- Photocopy of Diploma or Certification of Graduation authenticated by the Registrar of the school where he/she graduated (please present original for authentication)
- Consent to participate in outside of school activities + Php 200.00 (please see forms and attachments in the letter of acceptance from UPCM)
- For Regionallzation Program (RP) students: 3 copies of Acceptance to Serve & Assumption of Liability (ASAL) signed by the student, parents & surety + Php 100.00 Notarial Fee
- Three (3) copies of Return Service Agreement (RSA) signed by the student, parents & surety + Php 100.00 Notarial Fee.
ADMISSION SLIP shall be Issued only after receipt of all the above credentials

- May 14, 2009 - Physical/Medical and Dental Examination
FOR NON-UP MANILA GRADUATES
You are required to have your Dental Examination taken at the College of Dentistry from 8:00 am to 12:00 nn. You may opt to have your dental exam taken at any private hospital/clinic provided that your examiner fills out the FORM REQUIRED ISSUED BY THE COLLEGE OF DENTISTRY. Submit this form to the College Secretary of the College of Dentistry. (**The College of Dentistry of UP Manila is located near the intersection of Pedro Gil Street and Taft Avenue, the nearest entrance to the College of Dentistry is at the gate across Pilar Hidalgo Lim Street corner Pedro Gil Street [landmark: ALVA Computer Shop])
PHYSICAL/MEDICAL EXAMINATION
UP Manila Graduates can directly proceed here on May 14, 2009. For non-UP Manila graduates, it is adviseable that you accomplish your Dental Examination prior to your Physical/Medical Examination since the College of Dentistry will only accommodate students from 8:00 am to 12:00 nn while the Physical/Medical Examination will last from 8:00 am to 4:00 pm.
IMPORTANT: Chest X-ray and Hepa-B Screening Results are required to be presented during the Physical/Medical Examination. The UPCM and PGH advise that you have your Chest X-ray and Hepa-B Screening at least a week before May 14, 2009, may it be at PGH or at any acceptable hospital. (**Venue for the Physical/Medical Examination is at the UP Manila Health Services, PGH; it is located at the central atrium of PGH, the simplest way to locate the Health Services is upon entering the PGH from the main lobby [the entrance with the 5 huge PGH murals], walk straight to the atrium stage entrance, directly facing the stage is a hall leading to the Health Services of the PGH)

- June 1, 2009 - ENROLLMENT
Form 5 will only be issued upon presentation of the ADMISSION SLIP, PE RESULT and properly filled-out IMMUNIZATION INFORMATION RECORD FORM.
Here is a general outline of the enrollment process:
- Filing of Form 5 completely and legibly including the reverse side
- Advising
- Checking
- Assessment (Students with special privileges, such as Faculty/Employee dependents, STFAP grantees, U.P. government scholars, students with loans, and applications for staggered payment, will proceed to the Registrar's Office, UP Manila, Padre Faura St. for re-assessment. It is adviseable for Faculty and Employee dependents to accomplish the necessary certifications from the RIDO UP Manila within days prior to enrollment. The RIDO UP Manila is located at the 8th floor, Central Block, PGH (take the PGH elevator up to the 7th floor and take the stairs to the 8th floor, a security guard will greet you there and assist you in locating the RIDO)
- Payment of fees will be accommodated at the College of Public Health Cashier's Office. Pedro Gil St., Manila or at the Cashier's Office, Padre Faura St., UP Manila. Personal checks will not accepted.
- Present your receipts of payment/s for recording at the Registration Area. Ask for a copy of your schedule of classes.
- June 2, 2009 - Orientation to UPCM (parents of students are advised to attend)
Time and Venue: 9:00 am to 11:00 AM at the BSLR-EAST and 1:00 pm to 3:00 pm at the UPCM Grounds
- June 8, 2009 - Start of Regular Classes in the UPCM
- June 9, 2009 - Welcome Ceremonies for Freshmen
